Our Aftercare Planning

At The Mend we believe the recovery process doesn’t end after completing detox and residential treatment. Aftercare typically involves transitioning to an outpatient treatment program, coupled with some form of structured-living. Most of our residents discharge, door-to-door, into the care of one of our trusted aftercare treatment partners.

Discharge planning begins 1-2 weeks prior to the anticipated discharge date and typically involves an intake assessment with staff from the aftercare program. If a resident decides he or she doesn’t want to purse outpatient treatment, we will provide the resident with 12-step meeting schedules and other resources to continue recovery. Our team will stay in touch with residents following discharge to see how their recovery is progressing. We will work to ensure that each resident’s discharge plan sets him or her up for ongoing success in the recovery process.

Our Recovery Community

At The Mend, we pride ourselves on growing and utilizing a large recovery community here in Orange County. We maintain routine correspondence with program alumni and provide current residents with access to alumni of The Mend and other after-care programs we work with. Access comes in the form of weekly panels at the facility, social outings and off-site 12-step meetings where alumni commonly attend. We truly believe in the benefits of fellowship during all stages of the recovery process.

We understand that clients are in treatment to learn how to live their lives effectively and we do not expect them to have all the answers. We recognize relapse can be a part of recovery. We understand and accept that a critical element of treatment is educating the client in making effective and productive choices/changes, which is accomplished by focusing on positive rather than negative actions, attitudes, and behaviors.
